Date​ ​contract​ ​signed b. Total​ ​value​ ​of​ ​contract c. Summary​ ​of​ ​services​ ​rendered​ ​by​ ​contract 2. A​ ​copy​ ​of​ ​each​ ​of​ ​the​ ​contracts​ ​included​ ​in​ ​point​ ​1.
