A record of incidents that had previous warning signs that were not addressed in the past 5 years in your local authority . For example, how many students reported concerns before the incident took place, and what was done about those concerns when they were initially reported for the past 5 years for (a) primary and  (b) secondary school.  Information is not held.  The number of incidents are so few, I am still awaiting confirmation from schools to confirm if any previous warning signs were reported.
